Summary Plan Description Important Information The Employee Retirement Income Security Act of 1974 (ERISA) requires that certain information be furnished to each participant (or eligible participant) in an Employee Benefit Plan. This is your SUMMARY PLAN DESCRIPTION. Contributions to this Plan are made by the Participating Employers. Contributions are based on negotiated contribution rates as set forth in the Collective Bargaining Agreement.

The Plan Year starts on January 1 and ends on December 31, and consists of an entire Calendar Year for the purposes of accounting and all reports to the United States Department of Labor and other regulatory bodies. Relevant provisions of the Collective Bargaining Agreement, the names of the parties and its expiration date may be reviewed at the Administrative Office:
The Collective Bargaining Agreements are between any Teamster Local Union affiliated with Joint Council of Teamsters No. 40, or any other union approved by the Trustees, and the various Employers that have entered into labor contracts with such Unions. PLAN INTERPRETATION The Trustees shall have the power to construe the provisions of this Plan, including, but not limited to, full and final determinations as to all issues concerning eligibility for benefits.
